In a prospective, multicenter registry (GUSTO-IV), 1519 consecutive patients with ST-segment elevation myocardial infarction (STEMI) treated with primary PCI during 2000 to 2002 were studied. Of the 1519 patients, 1206 had complete follow-up (median follow-up, 5.8 years). Independent predictors of increased mortality were identified by using a multivariable analysis (Cox proportional hazard model). Mean age was 60 +/- 13 years, and median left ventricular ejection fraction was 51% +/- 13%. Of the 1206 patients, 46% were men, 82% had STEMI, and 27% had cardiogenic shock. Thrombolytic therapy was administered to 285 patients (24%). Median follow-up was 5.9 years. Noncardiac causes of death were relatively frequent and mostly due to malignancies. Six-month mortality was 8%. Risk factors associated with increased 6-month mortality by univariate analysis were as follows: left ventricular ejection fraction, age, cardiogenic shock, troponin T, C-reactive protein, diabetes, bleeding, and previous myocardial infarction or unstable angina pectoris. Cardiogenic shock (p = 0.02) and previous myocardial infarction or unstable angina pectoris (p = 0.008) were associated with increased 6-month mortality in a multivariable analysis.
